Frequently Asked Questions

What can I expect from an evaluation report?

Each evaluation report includes:

  • Referral Information & Guardian Concerns – A summary of the reason for referral and caregiver observations.

  • Background & Developmental History – Relevant medical, developmental, and educational history.

  • Summary of Testing Results – Findings from the evaluation, including strengths and areas of need.

  • Recommendations for Services – Suggested therapy frequency, goals, and next steps.

A hard copy and digital report will be provided during the 30-minute evaluation results review meeting. Reports can also be shared with doctors, school staff, or other therapists upon request; however, a signed release of records is required before sending any medical information to third parties.
